To submit a privacy request to DDQX Learning Institute, Inc., a Delaware C-corp doing business in Arizona and operating as DDQX Learning, email info@ddqxlearning.org with the subject “Privacy Request,” or call 480-648-4049 for help submitting a request. Written requests may also be sent to:

Do not include your password, complete payment-card number, government identification number, patient records, or unnecessary sensitive documents. We will explain any additional verification needed through an appropriate process.

Depending on the law that applies, you may ask to access or receive a copy of personal information; correct inaccuracies; delete information; restrict or object to processing; withdraw consent; appeal a denied request; or exercise rights concerning certain automated decisions.
You may also ask to opt out of sale, sharing for cross-context behavioral advertising, or targeted advertising where applicable, and exercise any applicable right to limit use of sensitive information. Requesting a category does not mean DDQX carries out every activity described by that category.
Identify the service or information concerned and the outcome you are requesting. You do not need to use a particular legal term for us to understand and review the request.
For a request that would disclose, change, or delete information, DDQX may need proportionate confirmation of identity or authority. We use the information reasonably needed for that purpose and do not require creation of a new account.
We do not apply identity-verification requirements to an opt-out where applicable law prohibits them. An authorized agent may act for you subject to the requirements applicable to that request.
A parent, sponsor, or institution does not obtain access to an adult learner’s private records solely because it paid for a service. Rights relating to a minor are handled according to the applicable law and participation arrangement.
We respond within the time required by the applicable law. If a permitted extension is needed, we will explain the reason and expected response date. We aim to acknowledge a request within five business days.
Some records may need to be retained for a legal obligation, security, an unresolved transaction or dispute, or another permitted purpose. If we cannot fulfill a request in whole or in part, we will explain the applicable basis to the extent legally permitted.
To request review of a denial, reply with the subject “Privacy Appeal.” Where an appeal right applies, DDQX will follow the relevant process and deadline. You may also contact your competent regulator. We do not unlawfully discriminate against people for exercising privacy rights.
Use the unsubscribe link in a marketing email to stop those promotional messages. You may still receive communications necessary for a purchased service, a transaction, or account security.
Visit Cookie Policy and Preferences for browser controls and available website choices. An email opt-out is not an immediate browser-level script blocker.
Where a legally applicable opt-out preference signal, such as Global Privacy Control, is received, DDQX is responsible for honoring it for the processing to which it applies. Use the privacy-request contact above to report a signal or control that does not appear to be working.
To correct the evidence used in a report or ask about an automated educational result, identify the assessment and explain the information you believe is inaccurate or incomplete. An educational disagreement and a personal-information correction may require different reviews.
